Abstract Background Popliteal artery entrapment syndrome (PAES) is an underreported and underdiagnosed condition that affects the lower extremities. Previous case reports mainly presented young uncomplicated PAES. Here, we report cases of complicated PAES in middle-aged patients. Furthermore, discuss importance a multidisciplinary team approach from diagnosis to treatment obtain favourable clinical outcomes against this rare disease. Case summary Two patients with recent claudication were diagnosed popliteal obstruction due At meeting, decided plan was prioritize gastrocnemius tendon accessory transfer surgical thromboendarterectomy. In patency unsatisfactory, additional on-site percutaneous intervention planned. Follow-up extremity computed tomographic angiography showed patent without any both two cases. Discussion vascular disorder. Clinical suspicion advanced imaging modalities can lead accurate diagnosis. A helps obtaining results using minimally invasive hybrid interventional treatments.
